The forms are accessible electronically (via google forms) for parents to read, acknowledge and sign.  Please download the emergency form and fill out all the required information.  When completing the emergency form, please enter information for both parents, if applicable, and at least 3 emergency contacts.  Return the form to your child's school.  Please complete the emergency form, even if there has been no change to contact information.  If you need paper copies of the forms, they are available at each school site. 

Completing these forms each year are necessary for the district/school to have the parents approval for different programs provided.

If there is a change to your home address, please make sure to send an updated proof of address along with the emergency form.
