SHARED RESIDENCY AFFIDAVIT
If you do not have a utility bill or lease agreement in your name, and are sharing or renting a residence, please fill out the Shared Residency Affidavit and provide the required documents listed on the form.  Any missing documents will delay your child's enrollment.
